Overview of ototoxicity and the International Ototoxicity Management Group

Content : In this round table, we will discuss the prevention and management of ototoxicity induced by agents such as aminoglycosides, cisplatin derivatives, and industrial chemicals. We will approach this topic from basic sciences to examples of successful ototoxicity management programs. Additionally, we will discuss the International Ototoxicity Management Group, which aims to unite researchers, scholars, and clinicians involved in ototoxicity, providing a platform for discussion and the development of initiatives to prevent, monitor, and treat ototoxicity in various populations. The session will include six short presentations followed by questions to guide the discussion on the role of audiologists in the prevention, monitoring, and management of ototoxicity induced by different agents.

Basic science of ototoxicity: Evidence from animal studies

Content : Ototoxicity, a critical adverse effect of certain medications, leads to hearing loss and balance disorders. This talk will delve into the basic science underpinning ototoxicity, drawing on robust evidence from animal studies. We will explore the molecular and cellular mechanisms by which ototoxic agents, such as aminoglycoside antibiotics and chemotherapeutic drugs, induce auditory damage. Key findings from rodent models will highlight the role of oxidative stress, mitochondrial dysfunction, and apoptotic pathways in ototoxicity. Additionally, we will discuss the implications of these findings for developing potential protective strategies and therapeutic interventions. This comprehensive overview aims to bridge the gap between basic research and clinical applications, enhancing our understanding of ototoxicity and paving the way for improved patient outcomes.

The effects of cisplatin and aminoglycosides on the human cochlea

Content : Cisplatin is a very effective platinum antineoplastic agent that is widely used for treatment of various types of cancers. Aminoglycosides and glycopeptides are very effective antibiotic families, frequently used for treatment of several bacterial infections. Common adverse events of these treatments are hearing loss, tinnitus, or vertigo. The cumulative incidence of cisplatin-induced hearing loss is higher in younger children, compared to older children and develops early during therapy. The reported occurrence of aminoglycoside or glycopeptide ototoxicity varies highly in children and seems to depend on the diagnosis, aminoglycoside subtype and use of co-medication. Tinnitus appears in childhood cancer survivors nearly three times more often than in siblings. Awareness about hearing loss and tinnitus in children that have been treated with cisplatin, aminoglycosides or glycopeptides is warranted. Age-specific audiological monitoring is important to identify hearing loss, tinnitus and vertigo timely. Much is still unknown about vertigo after cisplatin treatment.

Content : Hundreds of medications are known to cause auditory and/or vestibular dysfunction known as ototoxicity. Earliest detection of ototoxicity provides individuals and care teams the opportunity for identification of adverse effects, mitigation of hearing loss, and individualized interventions/accommodations. Preventing or minimizing auditory damage is critical to preserve quality of life for patients receiving treatment, promote informed decision making, and to reduce the longer term, societal impacts of hearing loss. This presentation will focus on the clinical monitoring and management of individuals at risk for treatment-induced auditory and vestibular dysfunction due to cisplatin and/or aminoglycoside antibiotics. Moreover, incorporating emerging approaches for earliest detection and prevention of ototoxicity into current practice to advance ototoxicity management will be highlighted.

Management of Ototoxicity in a Work Setting: a position statement from the International Ototoxicity Management Group

Content : The International Ototoxicity Management Group (IOMG https://www.ncrar.research.va.gov/ClinicianResources/IOMG.asp) is taking a multicultural and interdisciplinary approach to the ototoxicity management in specific medical, environmental and occupational contexts globally. The IOMG Environmental & Occupational Focus aims to contribute to the development and implementation of programs that identify and provide care to populations exposed to ototoxic chemicals at work or in their environmental. By leveraging evidence on the auditoty and vestibular effects of ototoxic chemicals we identified tools and processes tailored to workplace health programs. These inform 1) referral decisions for diagnostic testing and 2) the specific needs for evaluation and control of exposures.

Content : In South Africa, the management of ototoxicity has seen innovative approaches aimed at improving patient access and outcomes. The country’s efforts reflect a commitment to addressing healthcare challenges through (a) strategic, community-focused interventions, (b) multi-sectoral collaboration, (c) use of technology, (d) focused research endeavours and (e) local national ototoxicity management guidelines. This paper aims to highlight some of the novel and sustainable solutions employed to enhance ototoxicity management in South Africa.
